LTSS Service Coordinator - RN Telehealth

Location: Virtual full-time role for candidates residing in the New York, New Jersey, or Connecticut tri-state area; required in-person training and commuting distance to an hirify.global office may apply.

Salary: $41.84–$62.76 per hour in New York.

What you will do

• Conduct telephonic clinical assessments covering physical health, behavioral health, social services, and long-term services and supports.
• Manage member cases involving chronic illnesses, co-morbidities, disabilities, and high-risk complications.
• Develop, monitor, evaluate, and revise individualized care plans with members, caregivers, families, physicians, and interdisciplinary teams.
• Coordinate access to services, including authorizations, referrals, alternative levels of care, and waiver programs.
• Collaborate with Medical Directors, Physician Advisors, providers, and interdisciplinary teams on treatment plans and service issues.
• Assign, supervise, and review the work of LPNs/LVNs, social workers, and other non-RN clinicians in accordance with applicable law and contracts.

Requirements

• Current, unrestricted RN license in New York required.
• At least 3 years of experience working with individuals with chronic illnesses, co-morbidities, or disabilities in service coordination, case management, or a similar role.
• Residence in New York, New Jersey, or Connecticut is required.
• In-person training sessions are required, and candidates must be within reasonable commuting distance of a posting location unless an accommodation applies.
• State-specific certification may be required based on applicable law or contract.

Nice to have

• Bachelor's degree in health or nursing.
• Bilingual Spanish, Mandarin, or Cantonese.
• CHHA and/or Medicare experience.
