LTSS Svc Coord-RN Clinician (US) (RN Clinical Care Management)

Location: Virtual full-time role based in Kansas, with required in-person training and residence within commuting distance of a posting location; Wichita, KS and Topeka, KS are referenced.

What you will do

• Manage member cases within the scope of an active RN license.
• Perform telephonic and face-to-face clinical assessments covering physical health, behavioral health, social services, and long-term supports.
• Develop, monitor, evaluate, and revise individualized care plans with members, families, caregivers, physicians, and interdisciplinary teams.
• Identify high-risk complications, alternative levels of care, and applicable waiver programs.
• Facilitate service authorizations and referrals and resolve provider, claims, and service issues.
• Direct and oversee LPN/LVN, social work, and other non-RN clinicians involved in member care.

Requirements

• High school diploma or equivalent.
• At least 3 years of experience working with individuals with chronic illnesses, co-morbidities, or disabilities in service coordination, case management, or a similar role.
• Current, active, valid, and unrestricted RN license in the applicable state or states.
• Ability to travel to the worksite and other locations as necessary.
• State-specific certification may be required under applicable law or contract.

• MA or MS in Health or Nursing.
