LTSS Service Coordinator - RN Clinician (Plainview Texas)

Location: Plainview, Texas, United States. Candidates must reside in Plainview, be able to travel up to one hour away daily, and remain within reasonable commuting distance of the posting location. This is a field-based role with occasional office attendance; hirify.global associates are generally required to work at an hirify.global location at least once per week unless the role is specified as primarily virtual.

What you will do

• Manage member cases within the scope of RN licensure and coordinate care across the healthcare continuum.
• Perform face-to-face clinical assessments covering physical health, behavioral health, social services, and long-term services and supports.
• Develop, monitor, evaluate, and revise individualized care plans with members, caregivers, families, physicians, and interdisciplinary teams.
• Identify high-risk members, chronic conditions, disabilities, and opportunities for alternative levels of care or waiver programs.
• Facilitate service authorizations and referrals and help resolve provider, claims, and service issues.
• Direct and supervise LPNs/LVNs, social workers, and other non-RN licensed professionals involved in member care.

Requirements

• Registered Nurse qualification with a current, unrestricted RN license in the applicable state or states.
• At least three years of experience supporting individuals with chronic illnesses, co-morbidities, or disabilities in service coordination, case management, or a similar role.
• Ability to travel to worksites, client sites, and other locations as required.
• State-specific certification may be required based on applicable law or contract.

• MA or MS degree in health or nursing.
