LTSS Service Coordinator – RN Clinician (RN Case Manager) Eastern Ohio

Location: Field-based role in Eastern Ohio, with hiring in Richland and Franklin counties; candidates must reside within commuting distance of a posting location or office. The role involves travel to client sites and designated locations, with occasional office attendance for meetings or training.

Salary: $36.38–$54.58 per hour in Columbus, Ohio.

What you will do

• Perform telephonic and face-to-face functional assessments covering physical health, behavioral health, social services, and long-term services and supports.
• Manage cases for members with chronic illnesses, co-morbidities, and disabilities while coordinating care with members and healthcare teams.
• Develop, monitor, evaluate, and revise individualized, person-centered care plans.
• Set short- and long-term goals with members, caregivers, families, physicians, and other support networks.
• Coordinate access to services, including authorizations, referrals, alternative levels of care, and waiver programs.
• Collaborate with Medical Directors, Physician Advisors, interdisciplinary teams, and providers to resolve care, claims, and service issues.

Requirements

• High school diploma or GED equivalent.
• At least 3 years of experience working with individuals with chronic illnesses, co-morbidities, or disabilities in service coordination, case management, or a similar role.
• Current, active, valid, and unrestricted RN license in Ohio is required.
• Ability to travel to client sites and attend an office occasionally, based on role requirements.

Nice to have

• BA/BS degree in Health or Nursing.
• Case management experience with older adults or individuals with disabilities.
