CareBridge Advance Practice Provider, Nurse Practitioner

Location: Ohio, United States; virtual full-time work is available only for candidates within reasonable commuting distance of the posting locations, except for required in-person training sessions.

Salary: $110,080–$165,120 annually for candidates working in person or virtually in Cleveland or Columbus, Ohio.

What you will do

• Provide primary and urgent healthcare through telephone and telehealth visits for patients receiving home and community-based services.
• Develop and implement clinical care and treatment plans for adults with chronic and complex medical and mental health conditions.
• Collect patient histories, perform examinations, review diagnostics, identify care gaps, and prescribe medication within state authority.
• Address patients’ physical and psychosocial needs while educating patients and families about medications, nutrition, adherence, illness progression, and crisis prevention.
• Coordinate care, resources, and clinical decisions with company physicians, other providers, patients, families, and interdisciplinary team members.
• Maintain medical records and documentation in accordance with state regulations and company standards.

Requirements

• Master of Science in Nursing.
• Active national nurse practitioner certification.
• Valid, current, active, and unrestricted Family or Adult Nurse Practitioner license in Ohio, plus an active RN license.
• At least 2 years of experience managing complex care cases.
• Experience working with electronic medical records.
• Ability to work Monday–Friday, 8:00 a.m.–5:00 p.m. local time, with on-call coverage of 2–3 nights every 4–6 weeks and holidays.

Nice to have

• Medicaid registration.
• DEA registration or eligibility.
• Experience managing complex care for patients with developmental disabilities and chronic illnesses.
