Everyday Care Advanced Practice Provider (NP or PA) - Responsive Care

Location: Fully remote within the contiguous 50 states. Monday–Friday, 8-hour shifts; 8am–5pm MST/PST preferred, with EST/CST hours considered.

Salary: $120,000–$155,000 annual base salary, plus a performance-based bonus of $10,000–$20,000.

What you will do

• Conduct focused virtual assessments for patients managing hypertension, hyperlipidemia, coronary artery disease, diabetes, and related chronic conditions.
• Develop individualized diagnoses and treatment plans covering medication management, lifestyle changes, and monitoring.
• Manage prescription refill queues and centralized results workflows, including review, triage, and follow-up on laboratory and diagnostic results.
• Use digital portal chat and AI-assisted prescribing and triage tools, while providing feedback to improve clinical workflows and the electronic health information system.
• Identify risk factors, comorbidities, contraindications, adherence concerns, and medication side effects.
• Coordinate care with PCPs, specialists, pharmacy, clinical nursing, social work, family members, and caregivers.

Requirements

• Active, unrestricted RN and APRN licenses for Nurse Practitioners, or an active Physician Assistant license.
• At least 3 years of outpatient clinical practice experience and at least 2 years focused on primary care or a related subspecialty.
• Experience managing hypertension, hyperlipidemia, and cardiovascular disease prevention.
• Ability to obtain licenses in requested states within 90 days of hire and maintain required state licenses.
• Knowledge of clinical documentation, patient privacy, telehealth regulations, and applicable legal and ethical standards.
• Proficiency in English and Spanish is preferred.

Nice to have

• Experience in virtual care, centralized results management, or live digital/chat-based patient engagement.
• Experience with STARS/HEDIS measures, clinical comorbidity coding, and identifying care gaps.
• Experience with or strong interest in AI-enabled clinical workflows and cardiometabolic care.
