TL;DR
LTSS Service Care Manager - Behavioral Health (Mental Health and Care Coordination): Developing and coordinating holistic care plans for high-risk long-term care members with significant mental and behavioral health needs, with an accent on home assessments, crisis support, and community resource coordination. Focus on evaluating complex member needs, facilitating referrals and service authorizations, educating families and caregivers, and maintaining compliant clinical documentation.

Location: Hybrid role based in McAllen, Texas, with frequent in-home and other site visits in the Laredo and Eagle Pass service area.

Pay range: $27.02–$48.55 per hour.

What you will do

  • Evaluate complex and high-risk members with significant mental and behavioral health needs and develop or support individualized care plans.
  • Act as a liaison and advocate for members and families when coordinating with physicians, facilities, agencies, providers, and specialists.
  • Conduct frequent home and site visits to assess needs, collaborate with resources, and support service delivery.
  • Educate members, families, and caregivers on preventive care, treatment options, medications, referrals, benefits, and available services.
  • Coordinate medical, behavioral, and social resources, including housing, transportation, employment, meals, foster care, and activities of daily living.
  • Document care management activities, support service authorizations, maintain compliance with clinical guidelines, and identify quality improvement opportunities.

Requirements

  • Master’s degree in Mental Health or Social Work, or graduation from an accredited school of nursing.
  • 2–4 years of related experience.
  • Required license or certification based on state contract requirements, such as LCSW, LMFT, LMHC, LPC, or RN with behavioral health experience.
  • Experience supporting members with depression, bipolar disorder, schizophrenia, borderline personality disorder, PTSD, substance use disorder, self-injurious behavior, or psychiatric inpatient admissions.
  • Ability to perform recurring in-home and site visits within the Laredo and Eagle Pass service area.
