Field Care Manager, Behavioral Health

Location: Must reside in Illinois. Remote work with travel to regional facilities, members’ homes, providers, and hirify.global offices; travel may reach 75% of working time. Working hours are Monday–Friday, 8:00 a.m.–5:00 p.m. CST.

Salary: $65,000–$88,600 per year, plus eligibility for a bonus incentive plan.

What you will do

• Provide integrated behavioral health care management through personalized coordination, crisis intervention, peer support, and provider collaboration.
• Support members with serious mental illness, serious emotional disturbance, substance use disorders, and complex social needs.
• Conduct face-to-face and telephonic member engagement, assessments, care planning, and transitions of care.
• Lead interdisciplinary care team activities, facilitate meetings, and coordinate communication among providers and service coordinators.
• Connect members with medical, behavioral health, community, and social services while improving health literacy and outcomes.
• Maintain compliance with Illinois Department of Human Services, CMS, and NCQA requirements.

Requirements

• Residency in Illinois is required.
• Active Illinois LCSW, LMFT, or LCPC license; supervisee and provisional licenses are not accepted.
• At least two years of post-degree clinical behavioral health experience.
• Case management experience with complex SMI, SUD, and SED populations.
• Ability to travel to facilities and homes for face-to-face assessments; valid driver’s license, car insurance, and reliable transportation required.
• Experience with electronic medical records and Microsoft Word, Outlook, and Excel.

Nice to have

• Case Management Certification (CCM).
• At least three years of in-home assessment or care coordination experience.
• Experience with Medicare, Medicaid, dual-eligible members, managed care, or field case management.
• Knowledge of community health and social service agencies.
• Bilingual ability.
