LTSS Service Coordinator (Case Manager) Eastern Ohio

Location: Field-based hybrid role in Ohio; candidates must live in Cuyahoga, Franklin, Geauga, Huron, Lake, Lorain, Medina, Monroe, Noble, Richland, or Washington County and remain within reasonable commuting distance of an office. Occasional office attendance is required for meetings or training.

Salary: $26.19–$39.29 per hour, listed for Columbus, Ohio.

What you will do

• Manage service coordination for a designated LTSS caseload, including long-term services, behavioral health, and physical health needs.
• Conduct face-to-face assessments using standardized tools and motivational interviewing techniques.
• Lead person-centered planning, document member goals and preferences, and create comprehensive support and backup plans.
• Coordinate with Medical Directors, caregivers, families, providers, and interdisciplinary teams to address clinical needs and gaps in care.
• Submit utilization and authorization requests, report critical incidents, and support grievances, appeals, fair hearings, and state audits.
• Mentor new staff and contribute to training and process improvement initiatives.

Requirements

• BA/BS degree and at least two years of experience working with a social work agency, or an equivalent combination of education and experience.
• Residence in one of the specified Ohio counties and ability to commute to an office are required.
• Experience coordinating services for members with chronic illnesses, disabilities, or co-morbidities.
• Ability to document care goals, coordinate support networks, and comply with state and federal regulations.

Nice to have

• Case management experience with older adults or individuals with disabilities.
• BA/BS in health or nursing.
