Care Manager (RN)

Location: Remote role based in Ohio; candidates must reside within designated Ohio counties and complete local travel and in-person member visits.

Salary: $56,200–$101,000 per year

What you will do

• Assess member needs, barriers to care, and social determinants of health, then recommend personalized care plans.
• Develop and update ongoing care and service plans with members, families, providers, specialists, and community resources.
• Coordinate healthcare services and facilitate timely access to appropriate care.
• Conduct telephonic, digital, home, and other site outreach, including local in-person member visits.
• Monitor member status and progress, provide disease-process and benefits education, and address care gaps.
• Document care management activities and member information in compliance with state, federal, and payer regulations.

Requirements

• Degree from an accredited school of nursing or a Bachelor’s degree in Nursing.
• 2–4 years of related experience.
• Registered Nurse state licensure and/or compact state licensure.
• Must be authorized to work in the United States without current or future employment-based visa sponsorship.
• Must reside in designated Ohio counties and be able to perform local travel and in-person member visits.
