Care Manager, Telephonic Registered Nurse

Location: Remote Kentucky, United States. Remote work from a dedicated home office is required; occasional travel to hirify.global offices for training or meetings may be required. Training and work schedules follow Eastern or Central Time requirements.

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Support Medicare members with transitional care needs and complex chronic conditions by telephone.
• Assess physical, environmental, behavioral, psychosocial, and clinical needs.
• Develop, implement, monitor, and adjust individualized care plans and interventions.
• Coordinate care with members, families, interdisciplinary teams, and healthcare, community, and social service resources.
• Identify barriers and risk factors, applying interventions to prevent avoidable hospitalizations and emergency department utilization.
• Make independent clinical decisions in a structured, fast-paced telephonic care management environment.

Requirements

• Active, unrestricted Registered Nurse license in the state of residence with active Compact Multistate licensure and no disciplinary action.
• At least three years of acute clinical care experience, including case management, discharge planning, and adult patient education.
• Clinical expertise in chronic-condition assessment, care coordination, clinical intervention, and individualized care planning.
• Strong clinical assessment, critical-thinking, decision-making, communication, and relationship-building skills.
• Proficiency with Microsoft Word, Excel, and Outlook, including simultaneous navigation of multiple systems and applications.
• Ability to work the required Eastern or Central Time schedule from a dedicated, interruption-free home workspace and protect member PHI/HIPAA information.

Nice to have

• BSN or MSN degree.
• Managed care experience.
• Certified Case Manager (CCM) certification.
• Bilingual English and Spanish communication skills without limitations or assistance.
