Field Care Manager Nurse (Healthcare)

Location: Must reside in Illinois; hybrid home-based work with occasional onsite work at the Schaumburg market office and 75–90% local travel throughout Illinois. Schedule is typically Monday–Friday, 8am–5pm CST.

Salary: $78,200–$107,600 per year, plus eligibility for a bonus incentive plan.

What you will do

• Assess and evaluate member needs to support optimal wellness and care outcomes.
• Visit Medicaid members in homes, supportive living facilities, long-term care facilities, and other care settings.
• Create and update plans of care while continuously monitoring progress.
• Identify and resolve barriers affecting effective care delivery.
• Coordinate with internal and external stakeholders, community health organizations, and social service agencies.
• Support either MLTSS or LTSS based on business needs.

Requirements

• Must reside in Illinois.
• Active Illinois Registered Nurse license without disciplinary action.
• At least 2 years of in-home case or care management experience.
• Experience working with adults and knowledge of community health and social service resources.
• Strong communication, interpersonal, computer, electronic medical record, and Microsoft Word, Outlook, and Excel skills.
• Valid state driver’s license, personal vehicle liability insurance, ability to travel 75–90% of the time, and successful TB screening are required.

Nice to have

• Bachelor of Science in Nursing.
• At least 3 years of in-home assessment and care coordination experience.
• Medicare/Medicaid, managed care, health promotion, coaching, and wellness experience.
• Case Management or Motivational Interviewing certification.
• Bilingual English and Spanish skills.
