Care Manager, Registered Nurse, RN

Location: Remote South Carolina; occasional travel to hirify.global offices may be required for training or meetings.

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Assess and evaluate members' healthcare needs in a telephonic care-management environment.
• Guide members and families toward appropriate care and wellbeing resources.
• Identify and resolve barriers that hinder care.
• Monitor patient care through assessments and evaluations to support desired outcomes.
• Create member care plans when appropriate and apply strategies to manage physical, environmental, and psychosocial health issues.
• Follow established guidelines while independently determining appropriate courses of action.

Requirements

• At least 3 years of clinical acute care experience.
• Active Registered Nurse license in South Carolina with no disciplinary action.
• Comprehensive knowledge of Microsoft Office applications, including Word, Excel, and Outlook.
• Ability to work full time, 40 hours per week, in a telephonic care-management role.
• Dedicated, interruption-free workspace is required to protect member PHI and HIPAA information.
• Home internet must provide at least 25 Mbps download and 10 Mbps upload speeds.

Nice to have

• Experience with case management, discharge planning, and adult acute-care patient education.
• Managed care experience.
• Certified Case Manager (CCM) credential.
