Senior LTSS Service Care Manager RN

Location: Dallas, Texas — hybrid role with in-home assessments in South Dallas, including Oak Cliff, Lancaster, and DeSoto

Pay: $36.21–$65.09 per hour

What you will do

• Assess the needs of complex and high-acuity long-term care members and develop personalized care and service plans.
• Coordinate care among members, families, physicians, facilities, agencies, specialists, and community providers.
• Monitor member status, clinical symptoms, complications, care-plan progress, and potential higher-level-of-care or waiver eligibility needs.
• Educate members and caregivers on disease processes, care options, provider instructions, referrals, benefits, and available community resources.
• Coordinate service authorizations for services such as meals, housing, transportation, foster care, and activities of daily living.
• Document care-management activities and collaborate with leadership to improve quality, compliance, and cost-effective service delivery.

Requirements

• Graduate from an accredited school of nursing or a bachelor's degree with 4–6 years of related experience.
• Bachelor's degree in Nursing preferred.
• Current RN state licensure and/or compact state licensure, or current state NP licensure, required.
• RUG certification must be obtained within 90 days of hire.
• Ability to perform in-home and other site visits in the South Dallas service area.
