Government Programs Care Manager III - 743

Location: Remote, but must reside within a 60-mile radius of the Chicago office; travel within the assigned territory is required.

Base pay: $34.90–$57.89 per hour.

What you will do

• Provide telephonic and field-based care coordination for members.
• Inform and educate members about healthcare programs and their personal health needs.
• Engage members in discussions about adherence to individual health goals.
• Complete telephonic and field-based health screenings, comprehensive health assessments, and care plans.
• Support clinical operations through member and provider activities, including communication with physicians and external resources.
• Visit members' homes and facilities when needed to support complex and specialty populations.

Requirements

• Registered Nurse with at least 2 years of direct clinical care experience, or an accepted unrestricted behavioral health license for the relevant state.
• At least 3 years of wellness or managed care experience presenting clinical issues to members and physicians.
• Current, valid, unrestricted license in the state of operations or eligibility through reciprocity; compact RN licensees changing permanent residence must obtain active state licensure within 90 days of hire.
• Knowledge of the health and wellness marketplace, employer trends, and medical data analysis.
• Strong verbal and written communication skills for working with members, providers, internal staff, vendors, and community resources.
• PC proficiency with Word, Excel, PowerPoint, databases, and web-based applications; current unrestricted driver's license, transportation, applicable insurance, and willingness to travel are required.
