Manager, Care Management

Location: Remote from Illinois, with 50–75% field-based travel and occasional onsite meetings at hirify.global locations in Illinois. Typical schedule is Monday–Friday, 8:00 AM–5:00 PM CST, with flexibility based on business needs.

Salary: $86,300–$118,700 per year, plus eligibility for a bonus incentive plan.

What you will do

• Lead care management teams of nurses, social workers, behavioral health professionals, and support associates, with up to 15 direct reports.
• Oversee member needs assessments, care planning, service delivery, and resolution of social needs such as housing, food insecurity, and transportation.
• Develop care management models that use community health workers, peer support specialists, housing advocates, and other extenders.
• Build partnerships with community organizations, aging services, housing providers, public agencies, members, and families.
• Monitor program performance, analyze data, identify gaps, and drive improvements in quality, cost, efficiency, and customer satisfaction.
• Maintain policies, procedures, training, workflows, compliance, and cross-functional coordination across the care management program.

Requirements

• Must reside in Illinois.
• Associate’s degree or higher and an active Registered Nurse or Social Work license.
• At least 5 years of professional experience and 2 years of management or supervisory experience.
• Experience with Medicaid, Medicare, long-term care guidelines, benefits, policies, and care management models.
• Strong data analysis, communication, problem-solving, operational leadership, and collaboration skills.
• Valid state driver’s license, personal vehicle liability insurance, and ability to conduct extensive field-based travel.

Nice to have

• Bachelor’s or advanced degree in nursing, business, or a health-related field.
• Managed care experience and familiarity with Illinois Medicaid policies and systems.
• Experience with community health workers, peer support specialists, housing programs, or similar community-based services.
