Field Care Manager (Behavioral Health)

Location: Field-based in Kentucky, with quarterly meetings in Louisville, KY; travel required up to 75%, approximately 30–40 miles or 40 miles per day.

Salary: $65,000–$88,600 per year, plus eligibility for a bonus incentive plan.

What you will do

• Conduct comprehensive behavioral health and psychosocial assessments.
• Develop member-centered care plans with measurable outcomes.
• Coordinate services across healthcare providers, community resources, and support systems.
• Support reentry and care transitions, including discharge from incarceration.
• Conduct visits in homes, community settings, hospitals, shelters, and correctional facilities, alongside telephonic outreach.
• Identify barriers, monitor progress, and adjust evidence-based interventions to improve health outcomes.

Requirements

• Master’s degree in behavioral health or a related field.
• Active, unrestricted Kentucky behavioral health license, such as LCSW, LPCC, LMFT, or another independent license with at least one year of post-degree clinical experience; alternatively, an active unrestricted Kentucky RN license, bachelor’s degree in nursing, and at least three years of behavioral health experience.
• At least two years of case management or care management experience.
• Experience working with adult populations.
• Ability to work in a field-based role with up to 75% travel.
• Valid state driver’s license and personal vehicle liability insurance required.

Nice to have

• Experience with Kentucky Medicaid, managed care, high-risk populations, SPMI, or SUD.
• Certified Case Manager certification.
• Knowledge of motivational interviewing and trauma-informed care.
• Experience in behavior change, health promotion, wellness coaching, and member engagement.
• Bilingual English and Spanish communication skills, including reading, writing, interpretation, and document explanation in Spanish.
