What you will do

• Conduct initial in-person clinical assessments and coordinate service activation for new patients.
• Develop, implement, and manage individualized patient plans of care.
• Coordinate care through communication with physicians, interdisciplinary teams, and community resources.
• Provide ongoing coaching and education to caregivers to ensure best practices.
• Review caregiver notes daily to monitor changes in patient condition or environment.
• Ensure continuous compliance with regulatory standards and company policies.

Requirements

• Valid and active Registered Nurse (RN) license in the State of Connecticut
• Ability and flexibility to travel within the assigned geographic area
• Valid driver’s license and car insurance
• Diploma from an accredited nursing program
• Proficiency in common software and web-based applications
• Strong empathy, active listening, and customer service skills
