Behavioral Health Case Manager II (Behavioral Health)

Location: Virtual full-time role tied to New York locations; candidates must live within a reasonable commuting distance, complete required in-person training, and may visit members in person in New York.

Salary: $69,520–$125,136 per year

What you will do

• Manage complex behavioral health and substance use cases by phone and through home visits within the scope of licensure.
• Assess member needs using screening criteria and clinical judgment.
• Develop, monitor, and modify individualized care plans.
• Support access to appropriate, high-quality, cost-effective care.
• Coordinate internal and external resources and collaborate with providers.
• Serve as a resource for other behavioral health case managers and participate in cross-functional initiatives.

Requirements

• MA/MS in social work, counseling, or a related behavioral health field, or a nursing degree, plus at least three years of relevant clinical experience.
• Broad experience with complex psychiatric and substance use disorder treatment.
• Current active, unrestricted professional license such as RN, LCSW, LMHC, LICSW, LPC, LMFT, LMSW, or Clinical Psychologist, as permitted by applicable law.
• Experience in case management and telephonic or in-person coaching for members with complex psychiatric, substance use, or medical disorders.
• Managed care experience is required.
• Applicants must be licensed in New York State and able to work within commuting distance of the listed New York locations.

Nice to have

• Health coaching and motivational interviewing experience.
• Managed care or insurance experience.
• Knowledge of community resources in the New York City and Westchester area.
