Community Health Worker (Healthcare)

Location: Pennsylvania, working at home within the Pennsylvania territory; frequent face-to-face community work in Allegheny County

What you will do

• Conduct face-to-face outreach for preventive health, maternity, care transition, and chronic condition initiatives.
• Educate members, complete Health Risk Assessments, identify barriers, and refer members to care management and behavioral health services.
• Coordinate access to appointments and preventive services with members, providers, community agencies, and state organizations.
• Build supportive relationships with members and community centers, including organizations such as Head Start, WIC programs, shelters, and human services agencies.
• Develop educational materials and conduct provider and community training on EPSDT and other preventive health initiatives.
• Represent hirify.global at community events and report training outcomes to management and peers.

Requirements

• High School Diploma or GED.
• At least one year of experience in a healthcare-related customer service or marketing environment supporting member engagement.
• Customer service knowledge, strong written and oral communication, empathy, patience, and cultural sensitivity toward Medicaid members.
• Ability to solve problems independently, manage multiple tasks, work proactively, and collaborate with internal and external partners.
• Ability to understand and address barriers such as transportation, childcare, financial difficulties, limited healthcare knowledge, and distrust of the healthcare system.
• Travel requirement: 50%–75%; the position is primarily on the road and involves regular community-based work.

Nice to have

• Case management experience.
• Experience in the nonprofit sector or with the targeted community or population.
• Community Health Worker experience.
• Basic medical terminology background.
