Field Care Coordinator (Healthcare)

Location: Spring Hill, Florida, with 75–90% local travel across Hernando County

Salary: $60,000–$65,000 per year based on experience and qualifications. The posted base-pay range is $53,700–$72,600 per year.

What you will do

• Visit Medicaid members in homes, assisted living facilities, long-term care facilities, and other care settings.
• Assess member needs, barriers, and requirements and develop individualized care plans.
• Plan and implement interventions to support member wellness and goals.
• Coordinate services with providers, payer sources, interdisciplinary teams, families, and community resources.
• Monitor and evaluate case management plans and adjust services as needed.
• Provide health education, motivation, and encouragement to members and families.

Requirements

• Bachelor’s degree in Health Services, Social Work, Psychology, or a related field and at least 2 years of relevant experience; or Florida-licensed RN with at least 2 years of relevant experience; or Florida-licensed LPN with at least 4 years of relevant experience.
• At least 2 years of case management experience and prior experience with Medicare and Medicaid recipients.
• Monday–Friday schedule, 8:30 a.m.–5:00 p.m.; 40 scheduled hours per week.
• AHCA fingerprinting and TB screening are required.
• Valid state driver’s license and proof of personal vehicle liability insurance are required.

Nice to have

• Bilingual English/Spanish communication skills.
• Nursing home diversion or long-term care case management experience.
• Experience with electronic case notes, geriatric populations, health promotion, coaching, and wellness.
• Knowledge of community health, social service agencies, and other community resources.
• 701B Certification or medication application (RFA) experience.
