Nurse Care Manager II (Bilingual Spanish Preferred)

Location: Virtual full-time role for candidates within reasonable commuting distance of the California posting location; required in-person training may apply. Schedule: 9:00 AM–6:00 PM PST.

Salary: $41.84–$52.30 per hour for positions in California.

What you will do

• Provide case management to health plan beneficiaries by assessing needs and supporting improvements in functioning and health outcomes.
• Identify appropriate medical needs, connect members with resources and services, address social determinants of health, and facilitate safe care transitions.
• Conduct clinical assessments, develop individualized goals and care plans, and coordinate their implementation.
• Monitor care plan effectiveness and modify plans as needed.
• Coordinate care with internal and external partners and participate in or lead departmental projects and initiatives.
• Obtain licenses in multiple states when required to support members across the country.

Requirements

• High school diploma or equivalent.
• At least 3 years of acute care clinical, condition-specific clinical, home health, or discharge planning experience, plus at least 1 year as a Nurse Care Manager I or equivalent experience.
• Current, unrestricted RN license in the applicable state or states.
• Multi-state licensure is required when providing services in multiple states.
• Associate or bachelor's degree in nursing and case management certification are preferred.
• Bilingual Spanish, managed care experience, and knowledge of medical management processes and member benefits are preferred.
