Outreach Care Specialist (US)-7

Location: Tampa or Miami, Florida, United States. Hybrid schedule with one required in-office day per week; candidates must live within reasonable commuting distance of the posting locations.

What you will do

• Conduct proactive member outreach by phone and through community-based engagement.
• Assess housing, employment, social-support, and access-to-service needs and identify barriers.
• Connect members with community-based organizations, housing resources, employment programs, and social-service agencies.
• Coordinate referrals and follow up with members and community partners to support successful access to services.
• Provide ongoing case management, track progress, address new barriers, and maintain accurate documentation.
• Collaborate with care-management teams and community partners to identify recurring barriers and improve member outcomes.

Requirements

• High school diploma or equivalent plus at least one year of related experience, or an equivalent combination of education and experience.
• Experience or demonstrated ability in case management, care coordination, community outreach, or member- and patient-facing support.
• Strong oral, written, interpersonal, problem-solving, organizational, and follow-through skills.
• Ability to coordinate multiple member needs, referrals, resources, and follow-ups.
• Ability to build rapport and trust with members, including individuals facing barriers to accessing services.

Nice to have

• BS or BA degree in a related field.
• Experience supporting housing or employment needs and partnering with community-based organizations.
• Familiarity with housing, workforce development, employment, healthcare, or social-service programs.
• Bilingual or multilingual skills.
