Community Resource Coordinator II (Healthcare)

Location: Remote, with residence required in Kansas. Travel is required within Shawnee, Osage, Douglas, Anderson, Franklin, Butler, Chase, Lyon, Greenwood, and Coffey counties; occasional home and community organization visits may be required.

Pay range: $17.84–$28.02 per hour.

What you will do

• Connect members with community and care resources in a cost-effective manner.
• Coordinate community outreach resources and promote awareness of available services.
• Support members with resource inquiries, correspondence, educational materials, and community connections.
• Conduct non-clinical health, home safety, transportation, employment, and community resource assessments.
• Perform telephonic and in-person outreach, including outreach to hard-to-reach individuals and families.
• Support nurses and social workers through community education, informal guidance, and member assistance.

Requirements

• Must reside in Kansas.
• High school diploma or GED.
• 1–2 years of related experience.
• Working knowledge of social determinants of health barriers.
• Ability to travel within the assigned Kansas counties and make home or community visits.
• Ability to document community resources and comply with health plan policies and standards.
