Care Coordinator II (Healthcare)

Location: Remote in Arkansas, with residence in Arkansas or a border city required. This is a field-based role requiring home visits and 30% travel within Sebastian, Crawford, or Franklin County.

Pay range: $17.84–$28.02 per hour.

What you will do

• Conduct member outreach by phone and through home visits to discuss care plans, next steps, resources, concerns, and ongoing education.
• Coordinate care activities with members, caregivers, healthcare providers, community organizations, and other partners.
• Support member and provider inquiries related to care and service plans.
• Communicate with care managers and practitioners to facilitate services and continuity of care.
• Support service assessments, screenings, referrals, and documentation of member needs.
• Maintain member records in accordance with state, contractual, and regulatory requirements.

Requirements

• High school diploma or GED and 1–2 years of related experience.
• For Arkansas Total Care: a bachelor's degree in social science or a health-related field, or a high school diploma plus at least one year of care-coordination experience with clients with developmental or intellectual disabilities or behavioral health needs.
• Must reside in Arkansas or a border city and be able to travel throughout the assigned service area.
• Valid driver's license, 30% travel, recurring child and adult maltreatment checks, and drug screening are required.
• Experience supporting behavioral health and developmental disabilities populations is required for the field-based role.
