Community Health Worker - Allegheny County Territory (Pittsburgh, PA)

Location: Allegheny County territory, Pittsburgh, Pennsylvania; work is primarily in the community with 50%–75% travel. Conditions also indicate working at home in Pennsylvania.

What you will do

• Conduct face-to-face outreach for preventive health, maternity, care transition, and chronic condition initiatives.
• Educate members, complete Health Risk Assessments, identify healthcare barriers, and coordinate referrals to care management and behavioral health services.
• Help members schedule appointments, navigate benefits, and access healthcare and community resources.
• Build relationships with community centers, human services providers, state agencies, and other local organizations.
• Develop educational materials and conduct training on EPSDT and other preventive health initiatives.
• Represent hirify.global at community events and report training outcomes to management and peers.

Requirements

• High School Diploma or GED.
• At least 1 year of experience in healthcare-related customer service or marketing.
• Knowledge of customer service principles, member needs assessment, and service quality standards.
• Strong written and verbal communication, teamwork, independent problem-solving, organization, patience, and empathy.
• Cultural competency and sensitivity when working with Medicaid members and people with limited English proficiency.
• Ability to work face-to-face in the community and travel 50%–75%.

Nice to have
