Care Coordinator 1

Location: East St. Louis, Illinois; field-based role with 75–90% local travel and occasional onsite meetings in Schaumburg, Illinois. Work is performed remotely between field visits, with occasional travel to hirify.global offices for training or meetings. Schedule: Monday–Friday, 8:00 AM–5:00 PM Central Standard Time, with flexibility available.

Salary: $53,700–$72,600 per year.

What you will do

• Visit Medicaid members in homes, assisted living facilities, long-term care facilities, and other care settings.
• Assess members’ care needs and help ensure services are delivered in the least restrictive setting.
• Identify and resolve barriers to effective care and implement interventions based on member needs.
• Coordinate services with payers, providers, interdisciplinary teams, families, and community partners.
• Monitor care plans, member progress, and outcomes through assessments, data, conversations, and active care planning.
• Guide members and families toward appropriate care and wellbeing resources.

Requirements

• Bachelor’s degree in social sciences, social work, human services, or a related field, or an active, unrestricted Illinois LPN license plus one year of relevant experience.
• At least one year of experience in healthcare and case management.
• At least one year of experience working with Medicare and Medicaid recipients, long-term care services, HCBS, and managed care organizations.
• Intermediate to advanced computer skills, including Microsoft Word, Excel, Outlook, electronic information systems, and electronic medical records.
• Ability to travel throughout the region for face-to-face meetings with members, families, community partners, and care teams.
• Valid state driver’s license and personal vehicle liability insurance are required. Patient-facing employees must complete tuberculosis screening.

Nice to have

• Active, unrestricted Illinois LPC or LSW license.
• Bilingual or multilingual ability in English and Spanish, Arabic, Vietnamese, Amharic, Urdu, or another language.
