Bilingual Case Management Nurse

Location: Fully remote, but candidates must be based in the United States and able to comply with HIPAA and company policies.

What you will do

• Coordinate healthcare services and communication among providers to deliver comprehensive, patient-centered care.
• Assess patient needs and develop individualized care plans aligned with medical conditions and health goals.
• Advocate for patients by supporting access to appropriate services, treatment adherence, and continuity of care.
• Monitor patient progress and intervention effectiveness, adjusting care plans when needed.
• Document assessments, care plans, outcomes, and resource utilization accurately and generate status reports.
• Educate patients and families on medications, treatment plans, preventive care, self-care, and healthy lifestyles while providing emotional support.

Requirements

• Active, unrestricted Registered Nurse license in the state of residence; a Compact License is accepted for residents of compact states.
• At least 3 years of nursing experience in an acute care setting.
• Fluent written and verbal communication in English and Spanish.
• Strong organization, attention to detail, communication, and collaboration skills in a deadline-driven environment.
• Critical thinking and sound judgment in complex situations, including appropriate escalation.
• US-based location and compliance with HIPAA and company security policies are required.

Nice to have

• Compact State Nursing License.
• Case management experience.
• CCM, CCP, or another relevant certification, or willingness to obtain CCM within 18 months of hire.
