Bilingual RN Field Care Manager (Arabic or Chaldean) Medicaid HCBS/LTSS

Location: Must reside in Michigan, specifically Wayne or Macomb Counties, and be able to travel 75%–90% for in-home visits. Work is scheduled Monday through Friday, 8:30 AM–5:00 PM Eastern Time.

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Manage a caseload and complete member assessments in home, community-based, and telephonic settings.
• Provide clinical support for members with complex medical needs and develop or update individual care plans.
• Coordinate with interdisciplinary care teams, caregivers, primary care providers, and external community resources.
• Identify barriers to healthcare and help members access social, housing, educational, and other services.
• Complete medical complexity attestations and clinical oversight activities.
• Conduct InterRAI Integrated Health Care assessments and Nursing Facility Level of Care Determinations to support eligibility, care planning, and regulatory compliance.

Requirements

• Residence in Michigan, specifically Wayne or Macomb Counties, with the ability to drive within the assigned area.
• Active Michigan RN or APRN license, including NP or CNS, with no disciplinary actions.
• Fluency in English and Arabic or Chaldean, including the ability to speak, read, and write each language independently.
• At least 2 years of healthcare and/or case management experience, including HCBS, waiver programs, LTSS assessments, Medicaid regulations, and person-centered planning.
• Experience collaborating with interdisciplinary teams, providers, members, and caregivers and maintaining accurate, compliant documentation.
• Knowledge of community health and social service resources, plus experience with EMRs, web navigation, keyboarding, Microsoft Word, Outlook, and Excel.

Nice to have

• Bachelor of Science in Nursing.
• Experience with InterRAI IHC assessments and NFLOCD documentation.
• Experience in health promotion, coaching, wellness, or managed care.
