Bilingual RN Field Care Manager (Arabic or Chaldean) HCBS/LTSS

Location: Must reside in Michigan, specifically Wayne or Macomb Counties; field-based work with 75%–90% travel for in-home visits within an assigned 30- to 40-mile area. Monday–Friday, 8:30 AM–5:00 PM Eastern Time.

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan and a $5,000 sign-on bonus subject to applicable terms and conditions.

What you will do

• Manage a caseload and complete comprehensive member assessments in home, community-based, and telephonic settings.
• Provide clinical support for members with complex medical needs and develop, coordinate, and update individual care plans.
• Coordinate with members, caregivers, primary care providers, interdisciplinary care teams, and external resources.
• Conduct InterRAI Integrated Health Care assessments and Nursing Facility Level of Care Determinations.
• Address healthcare, housing, social, educational, and other service barriers while advocating for appropriate member outcomes.
• Complete medical complexity attestations, clinical oversight activities, and compliant documentation.

Requirements

• Residence in Michigan, specifically Wayne or Macomb Counties, with ability to travel for in-home visits.
• Active Michigan RN or APRN license, including NP or CNS, with no disciplinary actions.
• Fluency in English and Arabic or Chaldean, including the ability to speak, read, and write independently.
• At least 2 years of healthcare and/or case management experience, including HCBS, waiver programs, LTSS assessments, and Medicaid regulations.
• Experience collaborating with interdisciplinary teams and applying person-centered planning principles.
• Experience with electronic medical records, web navigation, keyboarding, and Microsoft Word, Outlook, and Excel.

Nice to have

• Bachelor of Science in Nursing.
• Experience with InterRAI IHC assessments and NFLOCDs.
• Experience in health promotion, coaching, wellness, or managed care.
