Focus on identifying changing patient needs, addressing barriers to care, escalating concerns to triage nurses, and maintaining compliance across a fully remote day-shift schedule.

Location: Fully remote; day shift, Monday through Friday

ChartSpan provides chronic care management services for patients with multiple long-term conditions and supports healthcare providers between office visits.

What you will do

Conduct monthly outbound care-management conversations with assigned patients.
Identify patient-specific problems, goals, interventions, barriers to care, and changes requiring care-plan updates.
Connect patients with relevant healthcare resources and services.
Maintain accurate clinical documentation, including medical history, medications, immunizations, allergies, surgical history, and family history.
Provide health education and escalate patient concerns to the triage nurse team.
Meet utilization, scheduled-call, quality, attendance, and scheduled-hours expectations.

Requirements

Current active NY State LPN license and active multistate license.
LPN/LVN degree from an accredited nursing program.
Ability to follow established clinical protocols and procedures.
Flexibility to switch tasks effectively and contribute to team success.
