LTSS Service Care Manager (Healthcare)

Location: Remote-NC; must reside in Iredell, Rowan, Alexander, Davie, Davidson, Yadkin, Catawba, Wilkes, Lincoln, or Stanley County, North Carolina. Field-based duties may include home and other site visits.

Pay range: $27.02–$48.55 per hour.

What you will do

• Evaluate member needs, available resources, and potential care outcomes.
• Develop and monitor personalized long-term care and service plans.
• Coordinate members, families, caregivers, healthcare providers, specialists, and community resources.
• Educate members and caregivers on service options, referrals, provider instructions, and healthcare benefits.
• Document member information and care management activities in compliance with state, federal, and payer requirements.
• Identify opportunities to improve quality of care and cost-effective service delivery.

Requirements

• Residence in one of the specified North Carolina counties is required.
• RN or LCSW certification is required.
• Bachelor’s degree and 2–4 years of related experience, or an equivalent combination of education and qualifying experience.
• Experience with LTSS and/or HCBS coordination, care delivery monitoring, and care management; two years is required for the North Carolina Standard Plan.
• Experience interviewing and assessing member needs, managing caseloads, solving problems, and locating community resources.
• Strong communication, customer service, interpersonal, collaboration, assessment, cultural competency, and technology skills.
