LTSS Service Care Coordinator (Long-Term Care)

Location: Granite City, Illinois; hybrid-remote with 50% local home visits. Preference for applicants located within 30 miles of Alton/Wood River in Madison County, Illinois.

Salary: $22.94–$38.79 per hour

What you will do

• Assess member needs, available resources, and appropriate care outcomes for physical disability and aging waiver members.
• Develop and maintain personalized long-term care and service plans.
• Coordinate members, families, caregivers, healthcare providers, specialists, and community resources.
• Provide support related to employment, housing, independent living, participant direction, justice, and foster care needs.
• Monitor care plans, member status, and outcomes while recommending updates based on changing needs.
• Conduct on-site member visits, provide education on services and benefits, and document activities for regulatory compliance.

Requirements

• Bachelor’s degree and at least one year of related experience.
• For Illinois physically disabled/elderly plans, qualify as an Illinois-licensed RN, hold a bachelor’s or master’s degree in a qualifying human services-related field, or be an LPN with one year of experience conducting comprehensive assessments and providing formal services for elderly members.
• Ability to perform 50% local home visits and work within the Granite City/Alton/Wood River service area.
• Ability to work Monday through Friday from 8:00 a.m. to 4:30 p.m. Central Time.
