LTSS Svc Coordinator - RN Clinician

Location: Knoxville, Tennessee, United States. This field-based hybrid role requires travel to client sites or designated locations, occasional office attendance, and residence within commuting distance of an hirify.global office. Associates are generally required to work at an hirify.global location at least once per week unless the role is designated primarily virtual.

What you will do

• Manage member cases within the scope of an active RN license.
• Conduct telephonic and face-to-face clinical assessments covering physical health, behavioral health, social services, and long-term services and supports.
• Identify high-risk members and coordinate care with members, caregivers, physicians, and interdisciplinary healthcare teams.
• Develop, monitor, evaluate, and revise individualized care plans, including short- and long-term goals.
• Facilitate service authorizations and referrals and help resolve provider, claims, and service issues.
• Direct and oversee LPN/LVN, social work, and other non-RN clinicians involved in member care coordination.

Requirements

• High school diploma or equivalent.
• At least 3 years of experience working with individuals with chronic illnesses, co-morbidities, or disabilities in service coordination, case management, or a similar role.
• Current, active, valid, and unrestricted RN license in the applicable state or states.
• Ability to travel to worksites and other locations as necessary.
• Ability to work within commuting distance of the posting location and meet the role's hybrid and field-based attendance expectations.

Nice to have

• MA or MS in health or nursing.
• State-specific certification, where required by law or contract.
