Care Coordinator (Behavioral Health)

Location: Remote, Roswell, New Mexico 88203; service delivery may be virtual or face-to-face based on contractual requirements

Salary: $50,225–$75,335 annual base salary, depending on location, skills, experience, education, and other permitted factors

What you will do

• Coordinate care for members with behavioral health conditions who require intensive interventions and oversight.
• Conduct comprehensive health-risk and needs assessments covering medical, behavioral, psychosocial, environmental, financial, and community factors.
• Develop, communicate, document, and implement individualized care plans and resource strategies.
• Monitor interventions and plans of care, measure effectiveness, identify care gaps, and adjust strategies when needed.
• Collaborate with members, families, clinicians, physicians, providers, legal representatives, and community resources through an interdisciplinary care team.
• Educate stakeholders, support members with care-related concerns, maintain external relationships, and generate care coordination reports.

Requirements

• 3–5 years of experience in social work, nursing, healthcare, or a related field; relevant experience may substitute for a degree.
• Experience in utilization management, quality assurance, home or facility care, community health, long-term care, or occupational health.
• Experience analyzing trends using decision-support systems and interpreting data.
• Business management skills, including cost-benefit analysis, negotiation, and cost containment.
• Knowledge of referral coordination with community and public or private resources.
• Valid in-state driver’s license and effective verbal and written communication skills.

Nice to have

• Associate or bachelor’s degree.
• Certified Case Manager certification.
• Licensed Clinical Social Worker certification.
• Registered Nurse license in a state or compact state.
