Focus on conducting in-person, telephone, and telehealth assessments, facilitating urgent interventions, and maintaining continuity of care across interdisciplinary and community partners.
Location: Nashua, New Hampshire, United States. Monday–Friday, 8:00 a.m.–5:00 p.m.; the role includes in-person patient assessments and care coordination.

What you will do
- Coordinate referrals to behavioral health providers and resources, supporting timely access and effective transitions of care.
- Conduct patient assessments using the nursing process in person, by telephone, and through telehealth platforms.
- Assess behavioral health crises and coordinate interventions, referrals, and follow-up care.
- Act as a liaison among patients, families, providers, community agencies, and health system partners.
- Support screenings, prescription refill workflows, prior authorizations, controlled substance agreement tracking, and insurance-related guidance.
- Document care, track patient needs, and support behavioral health initiatives using Epic and other clinical systems.
Requirements
- Associate Degree in Nursing from an accredited nursing program.
- Current Registered Nurse license in the State of New Hampshire.
- Current BLS/CPR certification.
- At least two years of professional nursing experience in a relevant clinical specialty.
- Behavioral health experience, with strong assessment, clinical decision-making, communication, and crisis-response skills.
- Proficiency with EMR systems, including Epic, and Microsoft Office applications.
Nice to have
- Bachelor’s degree in Nursing, Psychology, Sociology, Human Services, or a related field.
- Experience in ambulatory care, behavioral health, care coordination, case management, or population health.
- Certification in Ambulatory Care Nursing, Care Coordination, or a related specialty.
