Care Coordinator- Montgomery County

Location: Must live in Montgomery County, Tennessee, United States; remote designation with required travel to members’ homes for visits. Schedule: 8:00 a.m.–5:00 p.m. EST, with no on-call requirement.

What you will do

• Partner with members and families to identify supports and services that promote health, employment, and independent or community living.
• Collaborate with clinical and social support colleagues to address physical, behavioral health, and long-term service needs.
• Conduct face-to-face visits and assess physical, behavioral, functional, psychosocial, financial, employment, and independent-living needs.
• Apply criteria to authorize appropriate home and community-based services.
• Confirm that authorized services are delivered and members’ needs are being met.
• Manage competing requests and priorities through critical thinking, organization, and time management.

Requirements

• At least 2 years of clinical experience; 3 years in a clinical setting is preferred in the role description.
• Registered Nurse with an active Tennessee license, or an active license in the state of residence if participating in the Nurse Licensure Compact; alternatively, a Master’s in Social Work with an active unrestricted LCSW, LMSW, or LAPSW license.
• Must live in Montgomery County, Tennessee.
• Valid driver’s license required for home and community visits.
• Strong customer service, communication, interpersonal, organizational, and time-management skills.
• Basic PC skills, including Microsoft Office and email; digital literacy assessment required.

• Flexibility to work a compressed work week after one year, subject to management approval.
