CHOICES Care Coordinator (McMinn County)

Location: Remote in the USA, Tennessee; must live in McMinn County. The role requires travel to members’ homes for face-to-face visits and follows an 8:00 a.m.–5:00 p.m. EST schedule.

What you will do

• Partner with members and families to identify supports and direct services that advance health, employment, and independent or community living goals.
• Collaborate with clinical and social support colleagues to address physical health, behavioral health, and long-term service needs.
• Conduct objective face-to-face assessments covering physical, behavioral, functional, psychosocial, financial, employment, and independent-living needs.
• Use authorization criteria to arrange appropriate home- and community-based services.
• Confirm that authorized services are delivered and members’ needs are being met.

Requirements

• At least 2 years of clinical experience; 3 years of clinical-setting experience is preferred in the role description.
• Must live in McMinn County, Tennessee.
• Active Tennessee RN license, a qualifying Nurse Licensure Compact license, or an active unrestricted MSW license such as LCSW, LMSW, or LAPSW.
• Valid driver’s license; field coordinators must complete a TB skin test, and routine business driving requires compliance with the driver safety program, insurance, and motor vehicle record standards.
• Basic Microsoft Office and email skills, effective time management, strong communication, interpersonal, organizational, and customer service skills.
• Various immunizations or medical tests and a digital literacy assessment may be required.
