Care Navigator

Location: Remote from anywhere within the state of Missouri; Monday–Friday, 8am–5pm. May perform on-site visits as appropriate.

Salary: $22.94–$38.79 per hour

What you will do

• Assess member needs, barriers to care, available resources, and appropriate care outcomes.
• Develop and maintain personalized care plans and identify providers, specialists, and community resources.
• Educate pregnant members, families, caregivers, and care managers about care options, referrals, provider instructions, and benefits.
• Coordinate members and caregivers with healthcare providers to ensure timely access to services.
• Monitor care-plan progress and member status, collaborate with providers on revisions, and refer members for further care-management evaluation when needed.
• Collect and maintain member information and care-management documentation in compliance with state, federal, and payer requirements.

Requirements

• Bachelor’s degree and 2–4 years of related experience, or equivalent applicable knowledge and experience.
• Graduate of an accredited school of nursing if holding clinical licensure.
• Current state clinical license preferred; a Missouri LPN license is strongly preferred.
• Must be able to work remotely from within Missouri.
