RN Escalation Nurse (Remote Patient Monitoring)

Location: Fully remote within the United States; assigned coverage may include 1st shift, 2nd shift, or weekends

ChartSpan provides chronic care management and remote patient monitoring services for patients with multiple chronic conditions.

What you will do

• Serve as the RN-level escalation resource for chronic care management and remote patient monitoring populations.
• Assess complex patient concerns and out-of-range RPM readings using independent clinical judgment.
• Contact patients regarding urgent or complex clinical issues and determine appropriate interventions.
• Notify providers, refer patients to emergency services, or initiate immediate interventions when required.
• Coordinate with ordering providers, specialists, and care teams to resolve escalated concerns.
• Provide RN-level guidance to the LPN Escalation team, identify care-plan trends, and document assessments and interventions.

Requirements

• Current active compact RN license required.
• RN degree, ADN or BSN, from an accredited nursing program.
• Strong independent clinical judgment and the ability to make autonomous decisions in a fast-paced remote environment.
• Excellent communication skills for coordinating with patients, providers, and clinical teams.
• Ability to remain composed during high-acuity or time-sensitive situations.
• Willingness to work an assigned shift, including 2nd shift or weekend coverage.

Nice to have

• Experience in triage, telehealth, emergency departments, urgent care, or a similar setting.
