Field Advanced Practice Provider

Location: Field-based role in Visalia, California, with occasional office attendance and a requirement to reside within commuting distance of a posting location. Travel to client sites and designated locations may be required.

Salary: $115,584–$173,376 per year in California.

What you will do

• Provide home-based palliative care to adult patients with chronic and complex medical and mental health conditions.
• Develop and implement individualized clinical and treatment plans based on patient goals, health status, and current conditions.
• Perform patient histories, physical examinations, and diagnostic assessments as needed.
• Address patients’ and families’ physical and psychosocial needs with support from an interdisciplinary team.
• Educate patients and families about medications, side effects, illness progression, nutrition, adherence, and crisis prevention.
• Coordinate care and resources with physicians, providers, patients, families, and clinical team members while maintaining compliant medical records.

Requirements

• Master of Science in Nursing.
• Current unrestricted Nurse Practitioner or Physician Assistant license in the applicable state.
• DEA registration or eligibility and current CPR certification.
• Ability to work in the field and travel to patient or designated locations as required.
• Prescribing must comply with state authority, regulations, and company health standards.

Nice to have

• Two years of experience in home-based services, complex care, hospice, or palliative care.
• Bedside hospital, oncology, emergency room, or intensive care unit experience.
• Experience with electronic medical records.
• Bilingual English/Spanish skills.
