Field Advanced Practice Practitioner (Palliative Care)

Location: Field-based role supporting Lincolnton, Maiden, and Morganton, North Carolina; candidates must live within a reasonable commuting distance of the posting location and may need to attend an office occasionally.

What you will do

• Provide clinical and non-clinical palliative care services in patients’ homes.
• Develop and implement care and treatment plans for adults with chronic and complex medical and mental health conditions.
• Assess patients through history, physical examinations, and diagnostics as needed.
• Address patients’ and families’ physical and psychosocial needs with interdisciplinary team support.
• Educate patients and families about medications, illness progression, nutrition, adherence, and crisis prevention.
• Coordinate care, maintain medical records, complete continuing education, and prescribe medication as permitted by state authority.

Requirements

• Master of Science in Nursing required.
• Current unrestricted Nurse Practitioner or Physician Assistant license in the applicable state required.
• DEA registration or eligibility required.
• CPR certification required.
• Carelon Health roles require satisfactory completion of a tuberculosis test.
• Travel may be required.

Nice to have

• Two years of experience in home-based services, complex care, hospice, or palliative care.
• Bedside hospital, oncology, emergency room, or intensive care unit experience.
• Experience with electronic medical records.
• Bilingual English/Spanish ability.
