Field Adv Practice Provider (Palliative Care)

Location: Field-based role in Toledo, Ohio, supporting Fremont, Fostoria, Tiffin, and Norwalk; candidates must live within a reasonable commuting distance of the posting location. Occasional office attendance is required for meetings or training, and the hybrid workforce strategy may require onsite work at least once per week. Travel to patient homes and designated locations may be required.

What you will do

• Provide clinical and non-clinical palliative care services in patients’ homes.
• Develop and implement care and treatment plans for adults with chronic and complex medical and mental health conditions.
• Perform patient histories, physical examinations, and diagnostics as needed.
• Address patients’ physical and psychosocial needs in collaboration with an interdisciplinary team.
• Educate patients and families about medications, illness progression, nutrition, adherence, and crisis prevention.
• Coordinate care with physicians, providers, patients, and families while maintaining compliant medical records and prescribing medication within state authority.

Requirements

• Master of Science in Nursing.
• Current unrestricted Nurse Practitioner or Physician Assistant license in the applicable state.
• DEA registration or eligibility.
• Current CPR certification.
• Ability to work in home-based and field settings and comply with state regulations and company healthcare standards.

Nice to have

• Two years of experience in home-based services, complex care, hospice, or palliative care.
• Bedside hospital, oncology, emergency room, or intensive care unit experience.
• Experience with Electronic Medical Records.
