Field Nurse Practitioner / Physician Assistant (Healthcare)

Location: Akron, Ohio, United States; on-site field work within approximately a 55-mile radius of the assigned county

Pay: approximately $100 per completed assessment, plus paid mileage.

What you will do

• Conduct in-home Annual Wellness Visits and chronic condition assessments for diverse patient populations.
• Perform focused medical histories, physical examinations, vital-sign checks, medication reviews, and screenings.
• Complete screenings including Hemoglobin A1C, spirometry, KED, DRE, and FIT.
• Educate members about preventive care and chronic disease management.
• Document visits in the EMR to support value-based care and risk adjustment.
• Work independently within NP or PA scope of practice and applicable state regulatory guidelines.

Requirements

• Active, unencumbered NP or PA license in the applicable state is required.
• Ability to meet state-specific supervision or collaboration requirements for PAs.
• Three or more years of patient-care experience preferred.
• Comfort with EMR systems and independent field-based work.
• Ongoing commitment of approximately 30 hours per month and regular weekday availability.
• Ability to travel daily within approximately a 55-mile radius of the assigned county.
