Licensed Practical Nurse (LPN) - Memory Care

Location: Elgin, Illinois, United States; onsite night shift with every other weekend and varying days

Salary: Starting rate of $36.00 per hour

What you will do

• Provide licensed nursing services and administer medications in compliance with federal and state regulations.
• Complete resident admissions, assessments, discharges, care plans, service plans, and clinical documentation as needed.
• Track clinical conditions such as wounds and infection control under director guidance.
• Monitor resident health, notify physicians and family members of changes, and document communications.
• Respond to emergencies, provide health-related consultation and education, and coordinate with staff across departments.
• Maintain resident confidentiality and follow the Safety & Disaster Plan.

Requirements

• Training from an accredited licensed or vocational nursing program with general nursing education.
• At least one year of experience in senior care or a related field.
• Current Licensed Practical Nurse license in the state of practice.
• CPR certification required.
• Acceptable English proficiency for job duties, workplace communication, and emergency situations.
• Basic computer skills and the ability to work safely under pressure while handling multiple tasks.
