Certified Peer Support Specialist (Wellness & Recovery)

Location: Field-based role serving members in the Bronx, Brooklyn, and Queens, New York; candidates must live within a reasonable commuting distance of the posting locations. Occasional office attendance is required for meetings or training.

Salary: $23.31–$34.97 per hour

What you will do

• Coordinate care and care-management activities supporting members' wellness and recovery.
• Engage members and their families in building supportive recovery networks.
• Provide onsite psychiatric discharge-planning education at Recovery and Resiliency sites.
• Collaborate with Stabilization Teams on member advocacy, discharge planning, barriers, and service transitions.
• Maintain contact with assigned individuals and providers to assess members' responses to services.
• Support staff with decision-making and problem-solving.

Requirements

• Associate degree and at least two years of experience in health services, behavioral health, or peer support services, or an equivalent combination of education and experience.
• Valid, active recovery specialist/coach certification and/or peer/family support specialist certification is required.
• New York certification as a Certified Peer Support Specialist is required.
• Ability to visit members in person throughout the Bronx, Brooklyn, and Queens.

• Knowledge of care coordination and case management concepts.
• Bachelor's degree, BS, or MBA.
