Case Manager

Location: On-site in Longmont, Colorado, USA

Salary: $22–$26 per hour plus bonus structure

What you will do

• Provide individualized case management for patients with substance use disorders throughout their recovery.
• Orient patients to treatment services, policies, and expectations.
• Connect patients with internal and community resources through referrals and coordinated follow-up.
• Provide crisis intervention and support during critical situations.
• Assess social, educational, vocational, and recovery needs and develop, update, and maintain treatment plans.
• Support program implementation, process improvements, caseload management, and community outreach.

Requirements

• Bachelor’s degree or higher.
• Ability to meet applicable federal and state certification and licensure requirements.
• Availability for a Monday–Friday schedule from 5:00 a.m. to 1:30 p.m.
• Ability to work in a fast-paced clinical environment involving prolonged standing, sitting, bending, and frequent computer use.
• Ability to follow safety procedures and use personal protective equipment when required.

Nice to have

• Pursuing Certified Alcohol and Drug Counselor certification.
• Knowledge of addiction treatment, substance use disorder treatment methods, and opioid maintenance treatment trends.
