Bilingual Nurse Practitioner (Virtual Care)

Location: Primarily virtual full-time work for candidates with an active, unrestricted Nurse Practitioner license in Texas, Kansas, or Iowa and an active RN Compact license. Required in-person training may apply, and candidates must reside within commuting distance of a posting location, including Grand Prairie, Texas.

Compensation: $5,000 sign-on bonus.

What you will do

• Provide urgent healthcare through telephone and telehealth video visits for patients receiving home and community-based services.
• Develop and implement clinical care plans for adults with chronic, complex, and co-occurring medical and mental health conditions.
• Gather patient histories, perform assessments, order diagnostics as needed, and create treatment plans aligned with patient goals.
• Identify and close gaps in care while addressing patients’ physical and psychosocial needs.
• Educate patients and families about medications, side effects, illness progression, nutrition, adherence, and crisis prevention.
• Coordinate care with interdisciplinary teams and medical providers, maintain compliant medical records, and prescribe medication within state authority.

Requirements

• Master of Science in Nursing and active national Nurse Practitioner certification.
• Valid, current, active, and unrestricted Family or Adult Nurse Practitioner license in Texas, Kansas, or Iowa.
• Active, unrestricted RN Compact license required.
• Bilingual or multilingual skills required.
• At least two years of experience managing complex care cases.
• Experience working with Electronic Medical Records.

• Experience managing complex care for people with developmental disabilities and chronic illnesses.
