Behavioral Health Outreach Care Specialist Lead

Location: Field-based role covering Vancouver, Clark County, Klickitat County, and Skamania County, Washington, United States; occasional travel within the listed counties and occasional office attendance are required. Candidates must live within a reasonable commuting distance of the posting locations unless an accommodation is granted.

Salary: $30–$45 per hour.

What you will do

• Coordinate follow-up care plans by scheduling appointments and enrolling members in appropriate programs.
• Assess treatment-plan compliance by telephone and through on-site member visits.
• Identify barriers to compliance, coordinate resolutions, and recommend treatment-plan modifications or additional services with case management and providers.
• Connect members with local, state, and federally funded programs and coach them on reducing health risks.
• Document case updates, compliance information, and quality-related reports.
• Maintain agency relationships, serve as a team lead, and participate in cross-functional process-improvement initiatives.

Requirements

• High school diploma or equivalent and 5 years of related experience, or an equivalent combination of education and experience.
• Strong oral, written, interpersonal, problem-solving, facilitation, and analytical skills.
• Ability to work in the field across the specified Washington counties and travel to client sites or designated locations.
• Behavioral health or mental health experience.
• Certified nursing assistant or certified medical assistant credentials are preferred.
• A related bachelor's degree and bilingual skills are preferred.
