Community Health Partner (Healthcare)

Location: Massachusetts, United States — remote, with community-based member visits as needed

Salary: $27.40–$35.10 hourly

What you will do

• Collaborate with RN Care Managers and interdisciplinary care teams to support members with complex medical and behavioral health needs.
• Manage a member caseload and identify social, behavioral, and environmental barriers to optimal health.
• Develop and update care plans with members, supporting progress toward social and health-related goals.
• Conduct self-efficacy, condition-specific, and behavioral health screenings, including PHQ-9, GAD-7, AUDIT, and DAST-10.
• Coordinate provider appointments, diagnostic and lab testing, behavioral health referrals, and community resources to close quality gaps.
• Document interactions, track tasks, and communicate with members, care teams, and community partners using care facilitation and scheduling platforms.

Requirements

• 2+ years of progressive experience in care coordination, community health, health promotion, or a related field.
• Strong critical thinking, sound judgment, and the ability to identify barriers, collaborate, and escalate clinical concerns appropriately.
• High-quality written, verbal, and presentation communication skills.
• Previous experience with documentation.
• Familiarity with the communities served, lived experience, and commitment to supporting vulnerable and underserved populations.
• Must be able to work remotely from Massachusetts and meet members in community settings as needed.
