Field Care Manager; Behavioral Health

Location: Kentucky, with field visits in homes, community settings, hospitals, shelters, and correctional facilities; quarterly meetings in Louisville, KY

Salary: $65,000–$88,600 per year, plus eligibility for a bonus incentive plan

What you will do

• Conduct comprehensive behavioral health assessments of members’ behavioral, physical, environmental, and psychosocial needs.
• Develop member-centered care plans with measurable outcomes and adjust interventions as needed.
• Coordinate services across healthcare providers, community resources, and member support systems.
• Support care transitions and reentry, including transitions following incarceration discharge.
• Conduct field visits and telephonic outreach while monitoring member progress.

Requirements

• Master’s degree in behavioral health or a related field.
• Active, unrestricted Kentucky behavioral health licensure as an LCSW, LPCC, LMFT, or another independent behavioral health license with at least one year of post-degree clinical experience; alternatively, an active, unrestricted Kentucky RN license, bachelor’s degree in nursing, and at least three years of behavioral health experience.
• At least two years of case management or care management experience.
• Experience working with adult populations.
• Valid state driver’s license and personal vehicle liability insurance are required.
• Availability for Monday–Friday, 8:00 AM–5:00 PM work, overtime as needed, and up to 75% travel, approximately 30–40 miles.

Nice to have

• Experience with the Kentucky Medicaid population or managed care.
• Certified Case Manager certification.
• Experience supporting people with Severe and Persistent Mental Illness or Substance Use Disorder and other high-risk populations.
• Knowledge of motivational interviewing, trauma-informed care, behavior change, health promotion, wellness coaching, and member engagement.
• Bilingual English and Spanish communication skills, including reading, writing, interpretation, and document explanation in Spanish.
